Wellness Director- Crescendo Senior Living
Summary:
Responsible for planning, organizing and directing the overall operation of the Wellness Department in accordance with current state and local regulations, as well as with ICC policies and procedures.
Qualifications:
  • Supervisory: 3 years.
  • LPN Licenses.
  • Driver's License (Required).
  • Is a CNA with experience caring or overseeing the care of elderly or disabled individuals.
  • Possess previous experience providing care and supervision to the elderly.
  • Has completed required job training and keeps up to date on all required ongoing training.
  • Is 21 years of age or older.
  • Possess valid CNA certificate, state of California.
  • Possess a current first aid card and cpr card.
  • Obtained the required criminal clearance prior to employment.
  • Obtained the required health screening and TB clearance and is in good physical and mentally health, capable of performing assigned tasks.
  • Possess the knowledge and assessment skills to determine appropriateness of residents.
  • Possess experience in supervising staff.
  • Possess good communication skills both oral and written.
  • Able to interface with physician and families as needed.
  • Works well with all departments.
  • Possess adequate computer skills.
  • Possess the skills to work with electronic equipment such as: pagers, paging systems, email, cell phones, etc.
  • Possess a working knowledge of medication management.
  • Understands the rules, regulations and applicable laws pertaining to residential care.
  • Can direct and supervise others.
  • Possess good character with personal integrity.
Responsibilities:
  • As directed, assist in the coordination of resident admissions.
  • As directed, will ensure each resident’s physician report is complete.
  • As directed, will complete a resident pre-assessment to determine appropriateness prior to admission. This may include going out of the facility to area hospital, homes or other assisted living facilities.
  • As directed will prepare and complete a resident needs and service plan within 14 days after admission.
  • Reviews all resident needs and service plan on a quarterly basis or as needed basis if to address any change of condition.
  • Communicates with the resident’s physician as needed.
  • As directed handles any plans of action to correct deficiencies, complete medical forms, reports, evaluations, studies, charts, etc.
  • Oversees all caregivers
  • Ensures the facility medication room for all centrally stored resident medications is inaccessible to unauthorized personnel or residents.
  • Reviews resident medications to ensure there is a written physicians order for each.
  • Maintains communication with the pharmacies to ensure all resident medications are managed.
  • Adheres to all Title 22 regulation regarding incidental medical and dental care
  • Notifies the resident’s family and or physician regarding any changes in condition
  • Can administer first aid as needed.
  • Acknowledges all residents’ rights and will ensure staff does not violate those rights
  • Ensures that any suspected or alleged elder abuse such as neglect, or mistreatment of residents is reported.
  • Provides continuous staff training on prohibited, restrictive and prohibited health conditions.

Other Responsibilities:
  • Implements skin care prevention plan which includes skin assessments, charting and monitoring residents at risk for skin breakdown.
  • Can implement bowel and bladder-training programs for residents as determined by the pre-admission assessment or physicians report.
  • Will assess residents for change of condition and confers with the Administrator If resident requires higher level of care.
  • Works with the Administrator to make sure all caregivers and medication technicians receive initial and ongoing staff training.
  • Assists with the coordination of appropriate consultants and skilled professionals that are providing health services to residents.
  • Coordinates with vendors that provide resident medical supplies.
  • Available to respond to family member’s questions and concerns. 
  • Assures that the care needs of all Residents are met and maintained according to physician orders and ICC standards.
  • Participates in the interviewing and selection of Residents for admission, adjustments to care delivery/level and discharge.
  • Responsible for the hiring of Wellness team members, assignment of mentors and monitoring the work progress of new and existing Wellness team members.
  • Fosters a cohesive and collaborative work environment through mentor-ship and team member development.
